Career Opportunity : Career Coordinator SCM

SUMMARY
Under the general direction of the Program Manager. To provide Care Coordination/Case Management services to seriously emotionally disturbed children, adolescents and families. Carries out all job responsibilities with fidelity and accountability to the Agency’s Mission, Vision, Values, Guiding Principles and Strategic Plan.

RESPONSIBILITIES & DUTIES
1.0 Safety
• Exercises full compliance with the Agency’s Code of Conduct, all Agency policies and procedures and Federal and State laws and regulations. (non-violence, social responsibility)
• Reads and becomes familiar with program policy manual. (non-violence, social responsibility)
• Assists families in managing and planning for safety needs and creating individualized intensive support plans. (non violence, social responsibility)
• Provides crisis intervention/prevention for clients and families on a 24/7 basis. (non violence, social responsibility)

2. 0 Sustained/Future Positive Impact
• Participates in regular staff meetings, ongoing training and staff development provided by the Agency. (growth and change, social learning)
• Facilitates the development of a Child and Family Team Individualized Plan of Care. (open communication, emotional intelligence, growth and change)
• Provides linkage/referral to community resources for social, recreational, housing/basic needs, mental health, etc. (open communication, growth and change)

3.0 Cultural Sensitivity

4.0 Honor Individual Journeys
• Assess client needs for basic support (health care, mental health treatment, education); assessments include a comprehensive view of the client/family strengths, challenges, skills, supports, etc. (non violence, emotional intelligence, growth and change)
• Documents all client contacts and assumes responsibility for maintaining clinical records in accordance with Agency Standards. (open communication, growth and change)

5. 0 Fiscal Responsibility
• Assess service needs related to use of flexible funds; coordinates the use of service dollars as needed. (social responsibility, open communication)
• Participate in Blended Team Meetings (Teams of 3) once a month to review their case loads and review: Services Dollar use, Face-to-face contacts, share resources, discuss intakes, discharges, concerns, barriers and successes. (open communication, democracy, social responsibility)

6.0 Family Driven / Youth Guided
• Provides regular case management services to assigned caseload, which includes at least two face to face visits per month with a caseload of twenty youth/families. (growth and change, non-violence, open communication.)
• Provides advocacy and case management with educational programs, mental health providers, legal system and other service providers. (growth and change, open communication, non violence)
• Assures that clients and families obtain the services they need to assure their physical and mental health and the ability to live safely in their home community. (social learning, non violence)
• Introduces and educate family and team on the Child and Family Team (CFT) Process and assesses the fit between the CFT Process and the family needs. (open communication)
• Engages and educates the family’s team about the CFT Process. (open communication, growth and change)

7.0 Collaborative Relationships
• Arranges own schedule to be available to clients and families in their homes/community including working an evening if needed. (social responsibility)
• Develops/configures a CFT consisting of the family, as well as formal, informal and natural supports. (open communication, democracy, growth and change)
• Facilitates and monitors CFT meetings. (open communication, democracy, growth and change)

MINIMUM QUALIFICATIONS

EDUCATION: Associate’s Degree or Bachelor’s Degree in Human Services

EXPERIENCE: Associate’s Degree with two years experience or BS Degree with no previous experience

SKILLS: Working Knowledge of the CFT Process and Care Coordination. Ability to engage, plan, communicate and understand youth and families.

SPECIAL REQUIREMENTS: Valid NYS driver’s license in good standing and has use of an automobile-will be traveling Monroe County. Ability to work evening hours as needed.
